Enroll

Selecting Enroll Now! in the Login page opens the Enroll page. Fill in the required items and click the Continue button. Required information items include:

• Company Code (3 alpha characters and 3 numeric characters, total of 6)
• Location Code (5 numeric characters)
• First Name
• Last Name
• Group (Manager, Technician, Sales, or similar)
• Email Address (must be provided to receive notices of confirmation, test results, certificates of completion and other communication)
• State License Number (optional): State (two alpha characters) No. (12 alphanumeric characters) Categories: (provide 3 separate input fields, each with space for 4 alphanumeric characters)
• State License Number (optional)
• State License Number (optional) 
• User Login Name
• Password (please record your user login name and password in a safe place for convenient retrieval)

You may contact PT-U to obtain a forgotten password or to change your password.
Upon clicking to submit your enrollment, you will review and confirm your enrollment information, or click to begin again. On the confirmation screen, your complete company name, address and contact information will be displayed from PT-U records.
